Rationale 1: The best way to improve nursing care and patient outcomes when a client is not improving from the standpoint of the nursing regimen is to conduct a search for evidence that a different type of nursing action has been shown to be effective in improving client outcomes. Discussing nursing outcomes with a physician or student would not be examining nursing practice and ways to improve care. Reading a textbook will give the nurse accepted practice, but not the latest nursing research.
