Answer to Question 1

C
In response to identified concerns, the nurse may initiate discussion. Effective communication about sexuality requires caring, sensitivity, tact, compassion, the use of appropriate language, and a non-discriminatory attitude. It is important not to have preconceived notions about the clients' sexual identity or activity.
To assist the clients in their situational change, the nurse must explore communication and sexual patterns of the couple. Having similar parenting beliefs will have less impact on their sexual relationship.
To assist the clients in adjusting to the change of becoming a family, the nurse should explore their communication patterns. How long they have been married would be less significant.
The level of knowledge the clients have regarding healthy sexual relationships would not affect their sexual relationship as much as their ability to discuss their feelings with one another.

Answer to Question 2

D
The axillary site can be used for temperature measurement with newborns and uncooperative clients.
The rectal site should not be used for routine vital signs in newborns.
The oral site should not be used with infants.
Research has shown that disposable, single-use thermometers tend to overestimate true tempera-ture readings. The nurse should use the axillary site.
