Which of the following statements shows the greatest understanding of wound staging?
 
  1. An ulcer must involve broken skin in order to be staged.

An unstageable ulcer is a full-thickness tissue loss in which the base of the ulcer is covered by slough (yellow, tan, gray, green, or brown) and/or eschar (tan, brown, or black) in the wound bed. Until enough slough and/or eschar is removed to expose the base of the wound, the true depth and therefore the stage cannot be determined. The remaining options are not correct.

Granulation tissue is red, moist tissue composed of new blood vessels, the presence of which in-dicates progression toward healing. Soft, yellow or white tissue is characteristic of slough (stringy substance attached to wound bed), and you will need to remove this before the wound is able to heal. Black or brown necrotic tissue is eschar, which you will also need to remove before healing can proceed.
