The nurse observes for excessive suppression of the cough reflex (inability to cough effectively when secretions are present) or hallucinations with dosages that exceed recommendations of dextromethorphan. Kussmaul's respirations, dysrhythmias, and diaphoresis are not characteristic adverse effects.
