Answer to Question 1

C

Answer to Question 2

A
A. Pouches should be emptied when one-third to one-half full because weight of urine in pouch may weaken or dislodge skin seal.
B. Immediately after surgery, ureteral stents are in place and remain for up to 10 to 14 days. The physician then removes the stents.
C. Position client standing or supine and drape. Some clients may prefer to do pouch change while sitting because this may make it easier for them to see stoma. However, when skin barrier and pouch are applied in sitting position, skin may have folds and wrinkles.
D. Properly dispose of used pouch and soiled equipment according to facility policy.
