D. In the event of transfusion reaction, (1 ) remove tubing containing blood product and replace it with new tubing. Maintain patent intravenous (IV) line using 0.9 normal saline.

C. In the event of transfusion reaction, (6 ) monitor and document vital signs every 15 minutes or more frequently, if needed.
