Answer to Question 1

A
A. The most important and most basic technique in preventing and controlling transmission of infection is hand hygiene. Hand hygiene is a general term that applies to either hand washing, antiseptic hand wash, antiseptic hand rub, or surgical hand antisepsis. Hand washing refers to washing hands with plain soap and water
B, C, and D. Contaminated hands are a prime cause of transmission of infection. For example, a nurse caring for a client who has excessive pulmonary secretions assists the client in expectorating mucus and disposes of the tissues in a bedside container.

Answer to Question 2

D
If resistance to catheter insertion is encountered, have the patient take slow, deep breaths to promote relaxation while the catheter is slowly inserted. If resistance persists the patient may have an enlarged prostate or some other obstruction of the urethra.
