Fire
All employees at all job sites will be trained in fire protection procedures and response. The firefighting equipment necessary for each specific job site will be readily available and properly maintained. Emergency telephone numbers will be conspicuously posted.
Job Site Evacuation
In the event it becomes necessary to evacuate a job site, a loud continuous siren will be used to alert employees. The location of the safe area where personnel should collect for a headcount will be conspicuously posted and made known to all employees. Supervisors will be responsible for conducting a headcount of their direct reports at the safe area.

Answer to Question 2

How accident investigations are conducted and by whom should be explained in the company's comprehensive safety and health plan. A typical division of labor is to have supervisors conduct the investigations since they are closest to the work and to employees, but to provide supervisors with a standard form developed by a safety and health professional. A simple statement in the plan such as the following is all that is necessary:
Supervisors are required to conduct a detailed investigation whenever an accident occurs in their area of responsibility. Supervisors may and should contact the company's safety and health manager for advice and assistance. The standard accident investigation form is to be used.
