Answer to Question 1

The prednisone is causing an increase in blood glucose by glucose intolerance and possibly by creating substrate for gluconeogenesis by promoting muscle breakdown. While muscle is disappearing, fat is being produced from the excess glucose.

Answer to Question 2

Kcals to meet needs; protein intake 1.2 to 1.5 g/kg; Na restricted to 2 to 4 g/d; CHO should be about 40 to 50 of total kcals. CHO should be primarily complex with very little, if any, simple sugars since prednisone causes glucose intolerance. Fluids should not be a problem but may be restricted since edema is common with prednisone. Alcohol should be avoided. K usually is not restricted and may have to be increased to replace K lost by K-excreting diuretics.
