RK's lack of activity and excessive intake is causing his lean body mass to decrease and his adipose tissue to increase. His LBM to fat ratio is not going to be like that of a person who is active. Suggest a technique the RD could use to determine RK's actual body composition.

What does an elevated CRP and homocysteine indicate in general?

Answer to Question 1

Skinfolds measurements taken in at least three different places (five is better, seven is best) may be the only measurements available. This is more difficult with a bed-ridden patient who cannot follow commands, but is probably better than nothing. If available, bioelectrical impedance would work once RK has normal hydration.

Answer to Question 2

Elevated CRP indicates that inflammation is present in the body but does not identify where the inflammation is located.

Elevated homocysteine has been associated with numerous disease states, including atherosclerosis. However, it has not been established if homocysteine causes the disease state or if the disease state causes elevated homocysteine. Most articles indicate that elevated homocysteine is in some way associated with disease, particularly atherosclerosis, but the American Heart Association has not, at the time of this writing, declared it as a risk factor for CHD.
