On what basis did the MD decide that Mrs. R was anemic?

According to the IHDP Growth Percentiles for VLBW Premature Boys (obtainable through Ross labs; see references 3 and 19), how would you classify LV for height and weight for age at 5 months? What should his height and weight be at this age?
