Rationale: Bone health is often compromised in clients with end-stage renal disease because of long-term corticosteroid use placing them at risk for metabolic bone disease and bone fractures. The nurse should carefully assess their risk for falls to prevent injury. Because of malnutrition in these clients, they may have decreased muscle strength post-transplantation. As many as 33 of clients are hyperglycemic in the late post-transplant phase due to insulin resistance resulting from corticosteroids and other immunosuppressive drugs. Weight gain is common among clients recovering from renal transplant.

Long-term parenteral nutrition must be administered centrally because peripheral veins cannot handle high enough concentrations of nutrients to provide long-term nutrient needs. Intravenous nutrition refers to both central and peripheral parenteral nutrition.
