Answer to Question 1

 Low Hgb, Hct, mean cell volume, mean cell Hgb, and mean cell Hgb content
- Due to her low consumption of iron-containing foods (e.g. meat); based upon her usual intake, her daily iron consumption is about 6 mg per day (below the RDA)
 Elevated RBC distribution

 Urinary ketones
- Because Mrs. Williams is starving, her metabolism is altered and her production of ketones (to supply her body with fuel) is elevated due to her inadequate energy intake

 Inadequate oral intake
 Inadequate energy intake
 Altered nutrition related lab values

 Inadequate fluid intake
Below are examples of PES statements for several of the problems stated above.
 Inadequate oral intake related to oral motor dysfunction, limited food preferences, and prolonged time needed to finish meals as evidenced by reported energy intake of 60 estimated needs and decreased growth velocity..
 Altered nutrition related lab values (vitamin D) related to inadequate intake, increased vitamin D metabolism secondary to Phenytoin use, and decreased sunlight exposure as evidenced by 25-hydroxyvitamin D level(16 ng/mL) indicative of clinical deficiency.
 Inadequate fluid intake related to limited beverage intake between meals as evidenced by dry skin/mucous membranes, pt meeting 70 estimated fluid needs per 24 hour recall, and elevated BUN..
