An immediate transfusion reaction, often because of ABO incompatibility, is most often the result of human error. Erythroblastosis, whether because of ABO or anti-D, occurs as a result of a maternal-fetal incompatibility. A delayed hemolytic transfusion reaction occurs as a result of undetected alloantibodies, where the level of antibody was below the level of sensitivity at the time of transfusion.
