Position a spastic hand with the wrist in neutral position or slightly extended; fingers should be extended with the palm down or may be left in relaxed position with the palm up. Position the patient in supine position. Elevate the head of the bed 45 to 60 degrees. This increases comfort, improves ventilation, and increases the patient's opportunity to relax. Adjust the head of the bed according to the patient's condition. For example, those with increased risk for pressure ulcers will remain at a 30-degree angle. Position the head on a small pillow with the chin slightly forward. If the patient is totally unable to control head movement, avoid hyperextension of the neck. Too many pillows under the head may cause or worsen neck flexion contracture.
