A young client with SLE is upset because of a vacation that is scheduled that she will miss. The nurse tells the client that she should be able to go and plans to teach the client to:

  3. monitor kidney function while on vacation.
  4. take a daily pulse and record the results.

1. Sun exposure can increase damage to sensitive skin and result in serious infection.
2. Swimming is allowed, but the client should be instructed to keep the skin dry after swimming.
3. Kidney function is not an issue; skin lesions and infection are the priorities.
4. The client with SLE does not need to monitor the pulse.

1. Clients who resist the walker should not be told it is not that bad, because they see this use as a sign of further decline.
2. The nurse should present the walker or other assistive device in a positive light. The client should be informed that the walker will promote independence and allow him to perform activities that include weight bearing.

4. Written instructions will not convince the client to use the walker.
