Answer to Question 1

Answer: 2

1. The client exhibits signs of neurological impairment from a possible tumor. Edema in the legs would not be an expected assessment of a brain tumor.
2. Checking hand strength is an important objective assessment when neurologic alterations are suspected.
3. Peripheral vision is not a test the nurse does. The nurse would assess for blurred vision.
4. Hearing is not likely to be impacted by a tumor, but balance gait changes would be.

Answer to Question 2

Answer: 1

1. Clients can experience an inability to swallow when recovering from a CVA. The nurse should teach the client safety measures to prevent aspiration during meals.
2. The client in the hospital will not be driving.
3. Seizure is not usually a complication of CVA.
4. If damage to the eye is apparent, it is neurological, and glasses help with functional eye problems.
