Answer to Question 1

Answer: 2

1. It might be unrealistic to have a field trip in the evening to explore sea life.
2. SPF 15 lotion provides protection against the sun's most damaging ultraviolet radiation and would be the most practical of the suggestions.
3. There is no reason to stay out of the water.
4. Wearing long-sleeved shirts and long pants would be unrealistic if sea life is going to be explored.

Answer to Question 2

Answer: 3

1. The client is most likely experiencing cardiac tamponade, which is an emergency. A back rub is inappropriate.
2. Raising the bed might help the client to breathe better, but the priority is to notify the physician and the emergency care team.
3. This is an emergency, and notifying the physician and others, such as the code team, are the priority for the nurse.
4. Assessing urinary output will be important after the client has been stabilized.
