The nurse is managing the pain of a client who had surgery earlier in the day. The primary goal for pain management should be:

1. Pain is more difficult to relieve when it becomes severe, so the goal is to provide medication before the pain reaches the severe level.
2. The client is apt to be groggy and might not be able to ask for pain medication until the pain is very severe.
3. The doctor does not asses the client's pain level. The nurse is responsible for determining when the medication is needed.
4. It is the nurse's responsibility to assess the pain level and treat before the client or family asks for relief.

2. Morphine is apt to cause constipation.
3. Morphine can cause urinary retention.
