Reassessing the client's pain level after medication administration is considered evaluation and would be documented as E for evaluation and is numbered according to the problem. Interventions such as medication administration are labeled I and numbered according to the problem. The problem statement is labeled P and referred to by number. Progress notes are not part of the identified labels of PIE charting.

Answer to Question 2

Answer: A, B, E

Generation X, Generation Y, and Millennial nurses would all support the new policy and would ask if the nurses could e-mail each other and if the e-mails could be accessed from home. Veteran nurses may prefer communication to be personal or not attached to technology.
