The client's dry patches could be due to fluid deficit but would not require immediate notification. Two liquid stools a day is not excessive, but the client needs to take in enough fluid to account for the stools as well as the normal fluid needs of the body. Moisturizing the skin is a positive action of the client but does not indicate appropriate understanding of the teaching that is appropriate for this diagnosis.
