At the beginning of the second stage, the cervix is completely dilated and effaced. Maternal pushing facilitates the fetal descent occurring during contractions. During the second stage of labor FHR and uterine activity are evaluated every 15 minutes for low-risk clients and every 5 minutes for high-risk clients. Transition occurs following the active phase; both of which are during the first stage of labor. This is too early for pushing. Pushing should begin at the beginning of the second stage.
