1. Menstrual history is important for determining gestational age. Food allergies can impact nutritional intake, but most food allergies are not so extensive as to cause nutritional problems.
2. Family relationships are important to understand, specifically to find out what support systems are available to the teen. The location of the home is not important.
3. Asking a client directly what her body image is most often leads to a blank stare. Body image must be addressed more indirectly, by asking a client what she thinks about her body. But a client's body image is her belief, and cannot be corrected by the nurse. If the nurse assesses that the client has body image problems, working with the client on a longer-term basis is required.
4. Both of these are important to assess during pregnancy of teens, as both are fairly common among this age group.
