Answer to Question 1

1

Rationale:
1. Parents play an important part in fostering the child's self-esteem. Building on the child's strengths and talents and always pointing out the child's abilities increase the child's self-esteem.
2. In trying to build self-esteem in a child, it is important to talk about the child's abilities.
3. Complimenting a child's strengths is important to increase self-esteem.
4. Setting goals for the child to reach at a certain time could lead to disappointment if they are not reached on time. This will result in a drop in self-esteem.

Answer to Question 2

2
Rationale 1: The menstrual cycle appears to have started at a normal time, and so it is not the priority.
Rationale 2: The BMI for this client is too high, placing the adolescent at risk for cardiovascular disease, hypertension, and diabetes mellitus in later life. Therefore, nutritional intake is the most important topic to focus on with this client at this time.
Rationale 3: School performance is not as high a priority as the BMI.
Rationale 4: The mental health screening exam is not as high a priority as the BMI.
