A client is prescribed morphine sulfate 33 mcg/min by continuous infusion. The medication amount can be increased by 1 mg/hr for pain control if needed. An infusion bag containing 100 mg morphine sulfate/1 liter is being used.

Global rationale: First convert the dose from mcg to mg or 0.033 mg/min. Then multiply the dose per minute by 60 or 1.98 mg/hr. Then use the equation 1.98 mg/hr x 1000 ml/100 mg = 19.8 mL/hr. If 60 mcgtts = 1 mL then the client will receive 19.8 mL x 60 mcgtts = 1180 mcgtts/hr. Dividing by 60 minutes, the client will receive 19.8 or 20 mcgtts/min.
