Answer to Question 1

B
Grab bars in the bathroom can be installed to enable an older adult to get into and out of the shower or tub. Grab bars can also assist with balance during transfers. Light bulbs (response A) need to have increased wattage to promote vision for older adults. A Foley catheter (response C) is inappropriate simply to decrease toileting and ambulation. Padding the bed (response D) might be viewed as increasing functionality of the environment by preventing the whole bed from becoming soiled, but a toileting program, urinal or bedpan, or bedside commode would be more appropriate for increasing function.

Answer to Question 2

A
Older adults often make lists of things to do and hang calendars and reminder notes to aid their memory. Responses B, C, and D are incorrect; variety, change, new people, and new situations often increase confusion rather than stimulate memory.
