Answer to Question 1

Informed consent is the process of advising a patient about treatment options; depending on state laws, the provider may also have to disclose a patient's diagnosis, proposed treatment/surgery, reason for care, possible complications, likelihood of success, alternative treatment options, and risks if the patient does not undergo care. The Joint Commission requires that a patient consent to treatment, and that the record contain evidence of informed consent.

Answer to Question 2

Type of admission-demographic

Patient account number-demographic

Appendicitis-clinical
