Facility identification, including the name of the facility, mailing address, and a telephone number, must also be included on each report in the record.

Answer to Question 2

The patient registration form documents demographic, administrative, and financial data. The problem list documents the patient's diseases, conditions, allergies, and procedures. The medication list documents medications, dosages, associated diagnosis, and name of the physician ordering the medications. The progress notes document the initial history and physical examination and all subsequent visits. The ancillary reports document reports of ancillary testing completed in the office or by outside laboratories.
