Who is responsible for mandating automation of the MDS for all facilities participating in the Medicare and/or Medicaid programs?

Centers for Medicare and Medicaid Services (CMS)

Answer to Question 2

ICD diagnosis codes must be submitted for Medicaid, Medicare Part A SNF, and Medicare Part B claims. CPT codes are used for billing Medicare Part B services for rehabilitation and for physician visits to residents in a nursing facility.
