Answer to Question 1

The student's list should include most of the following fields: patient first name, middle name, last name, suffix, address, city, state, zip code, home phone, work phone, mobile phone, sex (or gender), date of birth, race (or ethnicity), next of kin or emergency contact, school or employer. They may also include similar fields for guarantor. Guarantor first name, middle name, last name, suffix, address, city, state, zip code, home phone, work phone, mobile phone. This exercise does not require them to enter insurance information.
