Answer to Question 1

B
Feedback:
Vials or prefilled syringes should be crystal clear (regular insulin or lispro) or milky white, depending on the type of insulin. Vials should not be shaken because this causes air bubbles to form, which could alter the dosage. The nurse should discard vials that do not easily resuspend when rolling them. Other signs of unusable insulin are frosting or coating on the bottle (especially with NPH) and any settling or clumping.

Answer to Question 2

B
Feedback:
During the period of the embryo, which lasts until the eighth week after conception, the embryo is in what is called the critical phase of human development. During these weeks, all the organs and structures of the human are formed and are most susceptible to damage.
