When a bandage is applied, the digits should be assessed by applying light pressure. If the area that was touched remains lighter, the skin is blanched and this indicates impaired circulation. The bandage needs to be released every 4 hours and not every 6 hours, unless directed otherwise. This is done to inspect the skin under the bandage. Toes should be warm and of the same temperature as the rest of the body. They should not be warmer. Body temperature need not be assessed every 2 hours unless the client's condition indicates it.
