Answer to Question 1

C
Feedback:
The client's house should be provided with small lights in darker areas to prevent accidental falls. The level of electrical outlets should be raised instead of lowered so that the client can reach them from a wheelchair without having to bend. Low coffee tables should be removed from the room to provide space for moving the wheelchair. The nurse should also instruct the client's caregiver to install hand-held showers instead of overhead showers mounted on the wall because they are much easier to use.

Answer to Question 2

C
Feedback:
For the newborn with transient tachypnea of the newborn, the nurse monitors oxygen saturation levels via routine pulse oximetry. Supplemental oxygen is given, if indicated, according to the health provider's order. Frequent vital signs are taken and the nurse observes nutritional intake and hydration levels. The infant with respiratory distress syndrome typically is transferred to a neonatal intensive care unit for complex, multidisciplinary therapies, including IV antibiotics, total parenteral nutrition, and administration of high concentrations of oxygen. Oxygenation using hood, cannula, or mechanical ventilation via ET tube is used according to the severity of symptoms. Continuous positive airway pressure (CPAP) treatments and exogenous surfactant by ET tube may also be included.

Your skin wrinkles if you stay in the bathtub a long time because the outermost layer of skin (which consists of dead keratin) swells when it absorbs water. It is tightly attached to the skin below it, so it compensates for the increased area by wrinkling. This happens to the hands and feet because they have the thickest layer of dead keratin cells.

It may be triggered by a virus, bacteria, or foreign body in the eye. Antibiotic eye drops alleviate bacterial conjunctivitis, and antihistamine allergy pills or eye drops help control allergic conjunctivitis symptoms.
