Answer to Question 1

A
Feedback:
It is vital to identify each client correctly in at least two ways before any care is given. Identification is done by the following: Checking the client's arm ID or asking the client his or her name and birth date. Do not ask the client if he or she is Mr./Mrs. Smith. Some clients will answer yes to anything and the nurse could have the wrong client. This is particularly true if the client does not speak the predominant language or if the client has dementia or a hearing disorder. In an emergency, ask another staff person to identify the client.

Answer to Question 2

B
Feedback:
To ensure the client's privacy, the nurse should use screens, curtains, and bath blankets in the client's unit. Turning radios and TV sets to a low volume and avoiding conversation in the vicinity of the client at night are steps taken to maintain a noise-free environment in the client's unit. Offering extra blankets to the client protects the client from catching a chill.
