Identification is done by the following: Checking the client's arm ID or asking the client his or her name and birth date. Do not ask the client if he or she is Mr./Mrs. Smith. Some clients will answer yes to anything and the nurse could have the wrong client. This is particularly true if the client does not speak the predominant language or if the client has dementia or a hearing disorder. In an emergency, ask another staff person to identify the client.
