Answer to Question 1

Ans: C
Feedback:
Similar to the nursing process, the process of care management ends with evaluation. Staff qualifications form the basis of the management process and delegation precedes evaluation. The input the manager receives from clients and their families is an earlier step in the process of evaluating the performance of the nursing staff.

Answer to Question 2

Ans: A
Feedback:
Privacy rules are the primary focus of the Health Insurance Portability and Accountability Act (HIPPAA), and just because the client in past admissions wanted her neighbors and family to know where she was in the hospital and update them on the information does not mean that she wants her private information handled the same way this admission. Every admission the client has to sign paperwork regarding the HIPPAA. The nurse's best response is to check the computer record and find out if the client still wants visitors and how to handle the private information. Providing the room number without checking first is risky because if the client changed how she wanted to deal with private information and did not want visitors or certain visitors, or information provided, then the nurse violated her rights. Stating that the client is not on the unit is a lie and is unethical.
