Answer to Question 1

Ans: B
Feedback:
When working with a visually impaired person, it is important to explain any actions clearly and ensure the person knows one's identity and role. It is likely unnecessary to have the person feel about the towels and cloths. It is not necessary to perform cognitive assessments or to speak loudly.

Answer to Question 2

Ans: B
Feedback:
Standing in front of the sunny window would make it difficult for the client to read the nursing student's lips. Sitting at the end of the bed in a chair would be too far away for the client to read the student's lips. It is not recommended to sit on a client's bed and it violates principles of personal space and infection control. By standing on the opposite side of the bed so the sunlight illuminates the student's face helps the client to read the lips and makes this the best choice.
