Answer to Question 1

Ans: B
Feedback:
CISD focuses on the present, is completed in one session, and requires an active nursing role (Ragaisis, 1994). Nurses can apply CISD to individuals, groups, and families. It usually takes place within 24 to 72 hours of the incident. Integrating the nursing process with CISD consists of seven phases: introductory, fact, thought, reaction, symptom, teaching, and reentry. The first five phases are part of assessment, the teaching and reentry phases are intervention, and evaluation takes place at the end (Ragaisis, 1994).

Answer to Question 2

ANS: 2
Assessment for commitment requires data collection from the client. This opening is the most neutral of the options given and the most open-ended; therefore it will be most likely to elicit a response. 1. The client may not understand the term. 3. Why questions usually elicit rationaliza-tion. 4. This does not seek information.
