It usually takes place within 24 to 72 hours of the incident. Integrating the nursing process with CISD consists of seven phases: introductory, fact, thought, reaction, symptom, teaching, and reentry. The first five phases are part of assessment, the teaching and reentry phases are intervention, and evaluation takes place at the end (Ragaisis, 1994).
