Answer to Question 1

ANS: 3
An outcome statement derives from the nursing diagnosis and is a projection of the expected in-fluence that nursing interventions will have on the client. It provides specific, measurable indica-tors by which to evaluate the results of nursing interventions. Option 3 is the only statement that refers to ineffective problem solving.

Answer to Question 2

ANS: 3
Option 1 is incorrect because the physician did not diagnosis incorrectly. 2. A positive variance shortens the length of stay. 3. A negative variance lengthens the stay, so this is correct. 4. There is no evidence that there has been damage from malpractice.
