Answer to Question 1

A
The nurse should not perform or encourage the patient to stretch all joints to their maximum limit. A 0 rating of muscle tone represents a complete absence of weakness. A 0 rating on flexibility represents a complete absence of stiffness and total flexibility. The nurse should warn the patient that there might be discomfort when moving a patient's joints and limbs while performing the musculoskeletal status assessment.

Answer to Question 2

B
If the evaluation data indicate a lack of progress toward goal achievement, the plan of care is modified. These revisions are made after reassessment of the patient, formulation of more appropriate nursing diagnoses, development of new or revised goals and expected outcomes, and implementation of different nursing actions or repetition of specific actions to maximize their effectiveness.
