A 0 rating on flexibility represents a complete absence of stiffness and total flexibility. The nurse should warn the patient that there might be discomfort when moving a patient's joints and limbs while performing the musculoskeletal status assessment.

If the evaluation data indicate a lack of progress toward goal achievement, the plan of care is modified.
