Repression is the unconscious blocking from awareness of any material that is threatening or painful. Projection is attributing one's thoughts or feelings to another.

The entire interdisciplinary team focuses on discharge planning but the social worker and the nurse have the greatest share of the responsibility. The social worker has knowledge of facilities and services. The nurse has knowledge of the patient's functional ability, cognitive and perceptual skills, and the understanding of the rationale for self-care skills.
