Answer to Question 1

D
Feedback:
Repositioning in a high Fowler's position can facilitate oxygenation in clients who are experiencing shortness of breath. A Venturi mask is used to deliver a precise mixture of oxygen and air to an acutely ill client. Oxygen by nasal cannula should not exceed 6 L/min. A side-lying position does not facilitate oxygenation.

Answer to Question 2

C
Feedback:
The nursing process is an organized sequence of problem-solving steps used to identify and to manage the health problems of clients. It is a way of thinking and not solely a group of tasks and it does not always lead to the curing of disease. The main goal of the nursing process is not the equitable distribution of finite resources, though this is often necessary.
