Answer to Question 1

ANS: D
IPV has no serious adverse effects, but local soreness may occur.
Very rarely, OPV has caused a mild form of polio, but the scenario discusses IPV.
The development of drowsiness and agitation is more often associated with DTaP.
A low-grade fever is a possibility post IPV vaccine but a high-grade fever is not a common side effect.

Answer to Question 2

Mild reactions are common and most often are manifested by a low-grade fever, fretfulness, drowsiness, and local reactions of swelling and redness.
At least 50 of children experience reactions.
