A. For clients with visual alterations, have pharmacy type larger labels on all medication containers.
B. Provide specially designed syringes with large numerals or syringe magnifier for clients with visual alterations.
D. For clients taking multiple medications, introduce a color-coding system. Colors could be used for drugs that are to be taken at the same time.

If signs of a transfusion reaction occur, stop the transfusion, start normal saline with new primed tubing directly to the ventricular assist device (VAD) at the keep-vein-open rate (KVO), and notify the physician immediately.
