A patient reports having an acute onset of low back pain associated with lifting a heavy object the day before. Besides advising the patient to rest and apply ice, the primary care NP should prescribe:
 
  a. an opioid analgesic.
  b. metaxalone (Skelaxin)
  c. cyclobenzaprine (Flexeril).
  d. a nonsteroidal antiinflammatory drug (NSAID).

Question 2

The primary care NP sees a patient who has heart failure following an MI 6 months before this visit. The patient has been taking an ACE inhibitor, nitroglycerin, furosemide, and hydrochlorothiazide .
 
  The NP auscultates crackles in both lungs and notes pitting edema of both feet. The NP should prescribe: a. mannitol.
  b. metolazone.
  c. acetazolamide (Diamox).
  d. spironolactone (Aldactone).

NSAIDs and acetaminophen are first-line analgesic treatments for low back pain. Opioids are used for severe low back pain. The other two medications are not first-line treatments.

Spironolactone has been shown to be of particular benefit in the treatment of severe congestive heart failure when added to an ACE inhibitor and a loop diuretic.
