Describe the relationships of Fosrenol, Aranesp, and Hectorol to ESRD.

Question 2

Today some are using a new glucose polymer, icodextrin, in peritoneal dialysis in place of glucose. Research icodextrin and describe the results reported thus far for this new polymer. In which patients might this be especially useful?

Answer to Question 1

Fosrenol: since phosphorous is not dialyzed well, the blood phosphorous level rises considerably. If phosphorous were to be restricted in the diet, with as many other restrictions that ESRD patients have, the diet would be even more restrictive. Instead a product like fosrenol is used to help prevent phosphorous absorption and thus control serum phosphorous.

Aranesp: with renal failure comes the inability to produce erythropoietin which results in anemia. Aranesp mimics erythropoietin, stimulates the production of red blood cells, and prevents anemia.
Hectorol: renal failure prevents the activation 1,25-dihydroxycholecacciferol which results in calcium absorption problems. Hectorol is a vitamin D analogue that stimulates calcium absorption and deposition in bone.

Answer to Question 2

Icodextrin: a glucose polymer in peritoneal dialysis solution. It is an osmotic colloid that is a starch-derived, water-soluble glucose polymer linked by alpha (1-4) and less than 10 alpha (1-6) glucosidic bonds.

This may be especially useful to diabetics.

Aldomet: an antihypertensive, centerally-acting.
