Answer to Question 1

Her target glucose was according to the recommendations of American College of Obstetricians and Gynecologists3 . Those levels are: fasting, 60 - 90 mg/dL; preprandial, 60 - 105 mg/dL; 1-hour postprandial, not > 130 - 140 mg/dL; 2-hours postprandial, < 120 mg/dL1 . Using the formula mentioned in the case, BS  100  20 = U of insulin to be administered, the results would be:
280  100  20 = 180  20 = 9 U of R insulin.

Answer to Question 2

With an ileostomy liquids are taken between meals instead of with meals as would be the case with a colostomy. Not as much emphasis on high fiber with the ileostomy. Emphasis on Na- and K-containing foods since electrolytes and water would be most affected with the ileostomy. Additionally, the ileostomy diet should be softer, bland foods.
