Describe the initial strength and flow rate you would use, the progression to the final flow rate, and the total kcals and protein Mrs. M would be receiving at the final flow rate (in total kcals and total grams and in kcals and grams per kg of IBW).

Question 2

Based on her recall, what would you recommend that Mrs. K eat during one of her attacks? Compare this to what you would recommend after the attack is over.

Answer to Question 1

Since Mrs. M has been eating and her ileus was a sudden one, the very slow progression probably is not necessary. The TF will be going into the small bowel, so it probably would not be good to start full strength unless an isotonic feeding is used. If a hypertonic feeding is used, start  strength and increase to full strength in 24 - 48 hrs depending on tolerance. Once at full strength and being tolerated, increase 10 cc every 8 - 12 hrs until the final rate is reached. The final rate, the kcals, and protein will depend on the TF used.

Answer to Question 2

During the attack Mrs. K should go on a low-fiber diet and eat only very soft, very easily digested foods, as discussed in the answer to question 7 . Depending on the severity of the attack, she may be on liquids for a few days. After inflammation has cleared, she should slowly return to a soft high-fiber diet as discussed in the answer to question 8 . A stool volume expander would also be helpful.
